If applicable, include the CAPP # , Schoolyard Improvement Project #, or any other supplemental documents.

IF MULCHING, THE BELOW QUESTIONS ARE VITAL FOR DELIVERY. HOWEVER, IF YOU DO NOT HAVE THAT INFORMATION NOW, YOU MAY ADD IT AFTER YOUR PROJECT HAS BEEN APPROVED.

Other examples: Trails, garden beds, landscaped areas. / Otros ejemplos: Senderos, camas de jardín, áreas ajardinadas
This person should theoretically be on site to meet the driver during delivery, but an accurate Google Map pin can make this unnecessary,
Please include anything that would inhibit a dump truck from safely dropping the mulch on your pin. For example, power lines, gates, tree limbs, etc.
This isn't set in stone and we can help you determine an amount, but a best guess now is appreciated! Come talk to us if you need help determining a proper amount of mulch.

For PARD parks - if you are planning to plant new trees or plants, you must have CAPP pre-approval. However, a CAPP is not required to replace existing plants. In order to receive approval, please provide the specific planting list of individual species for anything that you may end up planting, and indicate whether you have already discussed your plans with a District Manager, PARD Urban Forestry, or other division. If not on PARD land, please indicate whether this plan has been pre-approved (ex: if planting on school property, has principal or AISD representative pre-approved the project.)
